Abstract

Background and Purpose: Autism spectrum disorder (ASD) with a comorbidity of generalised joint hypermobility (GJH) is an emerging area of research. The international classification of function (ICF) provides a comprehensive tool for understanding the functional impact on patients with ASD and a comorbidity of GJH. To date, there is no research looking at balance and endurance training in patients with ASD and a comorbidity of GJH (ASD/GJH).

Case description: This case study describes a specific balance and endurance protocol on a 15 year-old-boy with ASD and a Beighton score of 8 out of 9. The duration of the protocol was 6 weeks. It was designed by a physical therapist. The study design used was a prospective intervention case report. Data points were collected at baseline, at Week 3, and Week 6. Outcome measures included the 6-minute walk test (6MWT), heart rate/ pulse oximeter, Borg score, and the movement ABC (balance section only).

Outcomes: At week 6 post-intervention, the child showed improvements in his 6MWT (467.83m at week 0 versus 660m at week 6). Improvement was also noted in his movement ABC (balance section only) score (9th percentile at week 0 versus 25th percentile at week 6).

Discussion: This case report shows that a specific balance and endurance training protocol has positive effects for the child with ASD/GJH. The Beighton score is a screening tool that can identify ASD patients at risk of GJH. This will identify patients that are at risk of injury, chronic pain, and disability. Providing balance and endurance training is anticipated to improve participation and function within the community and improve inclusivity within the child’s family activities.
