Abstract

This Master of Social Work portfolio, At the Intersection of Care: Bridging Systems Through Connection, Advocacy, and Hope, explores the development of an advanced integrated social work practitioner across clinical, organizational, community, research, policy, and leadership settings. Grounded in the Five Elements of Advanced Integrated Practice and social work competencies, the portfolio reflects the integration of knowledge, skills, values, critical self-reflection, and lived experience across micro, mezzo, and macro levels of practice.

Through practicum, coursework, and professional experiences, the portfolio examines trauma-informed and culturally responsive practice, anti-racism, diversity, equity, inclusion, belonging, behavioral health, and the importance of human connection in creating meaningful change. Featured work includes clinical practice with individuals and families, cross-system behavioral health care coordination through the Southwest Washington Youth Navigator Program, development of the K–12 Public Schools and Community Mental Health Collaborative, program evaluation and community needs assessment, behavioral health policy analysis and advocacy, and the development of a peer-to-peer mentorship framework grounded in collaboration, psychological safety, and shared leadership.

Collectively, these experiences demonstrate the interconnected nature of direct practice and systems-level change and the unique role of social workers in bridging gaps among individuals, families, communities, organizations, and systems. The portfolio ultimately reflects a professional identity rooted in curiosity, cultural humility, authentic leadership, advocacy, and a commitment to strengthening communities through connection, belonging, and hope.
